Medication Policy Information Forms Found at the Bottom of this Page


If your child needs to take a prescription medication during school hours, the school must have a physician signed permission form signed by the child's physician and parent. The medication must be in the prescription bottle with a current label. You can ask your pharmacist to provide two labeled bottles, one for school and one for home. The school nurse or trained designated personnel will dispense medications through the health office. For safety reasons, an adult must bring all medication to school

Epi Pens and Inhalers are prescription medications inhalers and will also need a physician signed permission form on file for your child to be able to use these at school.  Since these are emergency medicines, it is permissible for your child to carry them at school, with the consent from physician and parent (area to be checked on medication form). If your child is able to carry their emergency medication, we suggest that you also provide the school with a second inhaler/epi pen to be kept in the health office in case they forget their emergency medication at home.

Over the counter medications need to be in the original, labeled  bottle.  A permission form from the parent with medication and dosage needs to accompany the over the counter medication. The dosage amounts are not to exceed the recommended dosage on the bottle. The health office does not have a stock supply of Tylenol or Ibuprofen available for students.

Students in grades 9-12 are permitted to self carry non-prescription pain relief (acetameniphen or ibuprofen) medication if parents fill out and sign a medication authorization form. This completed from will be kept in kept in the health office. The dosages are not to exceed the recommended dosage on the bottle and must be kept in the original containers. New authorization/agreement forms will need to be submitted and signed each year.

All prescription AND over the counter medications will need to be picked up by parents or disposed of at the end of the school year. New doctor and parent permission forms will be required at the beginning of each school year.
